FACILITY USAGE REQUEST

Please fill out this form as a request for building use. Please keep these things in mind:

1. Form must be submitted at least 2 weeks prior to the event. Earlier is better.

2. Please do not plan your event until you have recieved confirmation that the facility is available for you.

3. Make sure to indicate the time you will arrive to prepare for the event and the time you will be locking up.

4. As the Event Contact, you are responsible for following the Shared Facility Use Covenant. Not following the covenant may result in extra fees paid directly to the facility overseer: Lee Quillen.
